A recent study suggests that gout may be hereditary rather than solely linked to lifestyle choices, overturning the belief that it only affected the wealthy in history, such as Henry VIII. Gout, the most common form of arthritis in men, has seen a significant rise in cases, with a staggering nearly 1,000% increase over just four years in certain areas of England. Symptoms of gout include sudden and intense pain in a joint, commonly the big toe, accompanied by red, swollen skin over the affected joint.

Experts from the University of Otago in New Zealand conducted a study involving genetic data from 2.6 million individuals worldwide. Their findings, published in the journal Nature Genetics, suggest that inherited genetics play a crucial role in determining why some individuals develop gout while others do not. Professor Tony Merriman, the senior author of the study, hopes that this research will help dispel the stigma surrounding gout by highlighting its genetic basis.

Merriman emphasised that gout is a chronic condition influenced by genetics and not solely caused by lifestyle factors or diet. While dietary choices like consuming red meat can trigger gout attacks, the root cause lies in elevated levels of uric acid, the formation of crystals in the joints, and an immune response attacking these crystals. The study uncovered numerous immune genes and pathways related to gout, opening up new avenues for preventing gout attacks and improving treatment options.

Currently, gout attacks are typically managed using non-steroidal anti-inflammatory drugs like ibuprofen, with steroids as an alternative if necessary. However, Merriman envisions better and more accessible treatments in the future based on the newly identified genetic targets. Repurposing drugs used for other immune-related conditions, such as tocilizumab for rheumatoid arthritis, could offer promising solutions for gout sufferers. Merriman advocates for increased healthcare resources and prioritisation for gout within the healthcare system to enhance patient outcomes.

As gout continues to affect a significant proportion of the population, advancements in understanding its genetic underpinnings could pave the way for more effective and tailored treatments to alleviate the pain and impact of this debilitating condition.
