A pregnant woman tragically passed away at Ninewells Hospital in Dundee due to a rare complication during labour. Jacqui Hunter died on May 13, 2020, after being admitted to deliver her baby, who was found to have died in utero the day before. An inquiry, led by Sheriff Jillian Martin-Bown, revealed that Jacqui suffered from an amniotic fluid embolism (AFE), a life-threatening condition that occurs when amniotic fluid enters the bloodstream during childbirth.
The inquiry disclosed that on May 12, 2020, Jacqui contacted Ninewells Hospital maternity triage reporting reduced fetal movements. Despite meeting with her community midwife the day before, who did not check the baby’s heartbeat, Jacqui felt a further reduction in movements prompting her to go to the hospital. An ultrasound confirmed the baby’s passing. Jacqui and her husband were offered an induction of labour which they agreed to. However, during the process, Jacqui received more misoprostol than the recommended dose, leading to complications.
On May 13, as Jacqui was about to receive an epidural, she fainted and later became unresponsive. Despite efforts to resuscitate her, she suffered a cardiac arrest and was pronounced dead at 4.29pm. A post mortem determined that the cause of her death was the amniotic fluid embolism. The inquiry found no evidence that the incorrect dose of misoprostol altered the course of events. Sheriff Martin-Bown expressed her condolences to Jacqui’s family, acknowledging the tragic outcome.
