New NHS guidelines as ‘resistant’ scarlet fever strain detected in the UK

A recent report from the UK Health Security Agency (UKHSA) has highlighted the detection of a ‘resistant’ strain of scarlet fever in the UK, prompting the issuance of new guidelines by the NHS to address the situation. The report pointed out a seasonal increase in cases of scarlet fever, particularly in the first three weeks of 2025, urging healthcare professionals to consider swift antibiotic treatments to prevent the escalation of infections. The guidelines emphasise the importance of timely antibiotic intervention to curb further transmission and reduce the risk of severe complications among patients and their close contacts.

Scarlet fever, caused by Group A streptococcal bacteria, can manifest with symptoms such as a high temperature, sore throat, swollen neck glands, and a distinctive rash that starts on the chest and stomach before spreading across the body. In more severe cases, the tongue may develop a ‘strawberry’ appearance. While scarlet fever is generally treatable with antibiotics, the detected strain in the UK, identified as the emm 49.8 gene style, has exhibited resistance to certain antibiotics like tetracycline and erythromycin.

To address the emerging threat posed by this resistant strain, national guidelines stipulate that both the UKHSA and Health Protection Teams (HPTs) should be promptly notified in case of suspected scarlet fever cases. Healthcare providers are advised to collect throat swabs for accurate diagnosis if needed, enforce exclusion from school and work until completing 24 hours of antibiotic therapy, and remain vigilant for potential complications or invasive Group A streptococcal infections.

The recent spike in scarlet fever cases, particularly in regions like London, the North East, Yorkshire, and the West Midlands, underscores the significance of implementing the new guidelines to mitigate the spread of the disease and safeguard public health. As the healthcare sector grapples with this evolving situation, a coordinated effort involving healthcare professionals, public health agencies, and the community at large is crucial to effectively manage the challenges posed by the ‘resistant’ scarlet fever strain detected in the UK.
