Dense breasts can make it harder to spot cancer on a mammogram

A new U.S. rule now requires mammography centers to inform women about their breast density, a crucial piece of information that can impact the effectiveness of breast cancer screening. Dense breasts can make it more challenging to detect cancer on a mammogram, as the dense tissue appears white on the X-ray image, much like cancer does, making it akin to “trying to see a snowball in a blizzard,” as described by Dr. Wendie Berg of the University of Pittsburgh School of Medicine.

Breast density classification varies across four categories, with “heterogeneously dense” or “extremely dense” falling under the dense category. While women with dense breasts are not at a higher risk of dying from breast cancer compared to others, they do face a slightly elevated risk of developing the disease due to the nature of glandular and fibrous tissue where cancers are more likely to arise.

If informed about having dense breasts, it is advised to discuss family history and potential additional screening options like ultrasound or MRI with a healthcare provider. While the gold standard for early cancer detection remains regular mammograms starting at age 40 for those at average risk, ongoing research aims to enhance ways of detecting cancer in women with dense breasts. Currently, there is insufficient evidence for a broad recommendation for additional screening, underscoring the need for further research in this area.

The cost of additional screening can be a barrier for many women, with out-of-pocket expenses ranging from $250 to $1,000. As discussions around insurance coverage for additional screening continue, the importance of equal access to early cancer detection remains paramount. Every woman should have the opportunity to have their cancer detected early when it is most treatable, emphasises Dr. Wendie Berg, underscoring the critical need for equitable and accessible healthcare practices in breast cancer screening.
