Forth Valley Health Authority Expresses Regret Over Shortcomings Resulting in Polmont Custody Deaths
The health chiefs of Forth Valley have issued apologies for the failures that led to tragic deaths in Polmont custody. The Chief Executive, Ross McGuffie, admitted to ongoing work since 2019 aimed at rectifying the shortcomings within health services.
The recent Fatal Accident Inquiry into the deaths of Katie Allan, 21, and William Brown, 16, also known as William Lindsay, in 2018 at Polmont Young Offenders Institution highlighted a series of deficiencies in healthcare services. Sheriff Simon Collins concluded that both deaths could have been prevented if not for a series of failures by the Scottish Prison Service and healthcare providers.
Addressing the NHS Forth Valley Board, Chair Neena Mahal extended heartfelt condolences to the families of Katie and William while acknowledging the healthcare services’ failures outlined in the report. She emphasised the need for continuous improvements to enhance healthcare provision to the prison population in the Forth Valley region.
McGuffie also conveyed his apologies and condolences, revealing that a review of mental health services at Polmont YOI in May 2019 resulted in 21 recommendations for immediate action. To date, 20 recommendations have been implemented, with one outstanding issue regarding information sharing protocols.
The recommendations encompass a diverse range of actions including enhancing supervision, staff training, timing of mental health assessments for high-risk individuals, and fostering multi-agency collaboration. Efforts are underway to fulfil the remaining recommendation.
Mahal underscored the importance of preventing such tragedies in the future, urging collective efforts to ensure the safety and well-being of individuals in custody.
